| Please download a copy of the MTA form to your local machine and mail us 2 signed originals providing your complete name and address with phone number, an express mail provider and account number (DHL, Federal Express, other) and indicate which plasmids you would like us to send. You will need to sign the forms, and obtain the signature of an authorized institutional representative for your institution such as your Technology Transfer Officer, or other responsible institutional officer. MTAs that are not completed properly will result in delays in sending the plasmids. | |
